S: AHEMS XX dispatched non-emergency, no lights & sirens to AH XX Lakes RM X for Pt. with a C/C of XX. Pt. has current complaint of XX. Pt’s current pain level is 0 out of ten with ten being the worst. Pt. has a PMH of none per RN. Pt’s meds & allergies as noted. Pt. denies any CP, SOB, headache, blurred vision, photophobia, dizziness, or N/V/D.  Report and paperwork taken from Staff RN.

O: AOS to find a XX y/o fe/male, semi-fowlers position, in their assigned bed.  Pt. was in no apparent distress but appeared anxious. AAOx4 with a GCS =15. Pt. had no visual trauma noted to the head with symmetrical facial features. Pupils PEARL at 3mm. Patent airway with no distress in speaking or breathing, & negative tracheal deviation or JVD. Pt. had clear & equal lung sounds bilat with symmetrical chest rise & fall. Abdomen was soft, non-tender x4 quadrants with no pulsating masses or rigidity noted. Back had no deformities, or trauma noted on palpation or visual inspection. Pelvis was intact and stable. Positive PMS noted in upper and lower extremities bilat. Cap refill <2 seconds. Skin intact, warm, dry, and pink in color at the mucus membranes. No deformities or trauma noted on exam. Pt. had a patent 20g IV Lock in the RAC.

A: Possible XX

P: Pt. transferred from bed to our stretcher self ambulation. Pt. seated in a semi-fowlers position of comfort, & secured with seatbelts x4 with side rails in the up position. Physical exam completed. Pt. escorted to ambulance via stretcher w/o incident. Pt. loaded in ambulance via the Stryker power-lift system & stretcher secured with locking mechanism. Pt. transported non-emergency, no lights & sirens to AH Winter Park RM 3401. Pt. was reassessed during transport, and was calm, & stable. Upon arrival at destination, Pt. was lowered from the ambulance via the Stryker power-lift system w/o incident. Pt. escorted to their assigned bed via stretcher. Pt. was assisted from stretcher to their bed by self ambulation, with the bed left in the lowest locked position with side rails up. Pt. report, & paperwork delivered to Staff RN.
